At Leeds Grenville Mental Health, we work hard to minimize wait times, so that we can get you the help you need, when you need it. We understand that time is of essence on matters of mental health, and will process your requests as soon as possible.
Our centralized intake is your SINGLE point of access for adult (ages 16+) community mental health services in Leeds and Grenville.
Referrals are accepted from family physicians, hospitals, mental health services, friends, family and self! By filling out the downloadable referral form below… you can gain access to services such as:
- Case Management
- Counselling
- Psychiatric Consultation (you must be referred by your family physician)
- Social and Recreational Programs
- Supportive Housing
- Vocational/Educational Programs

Take advantage of our innovative Open Access Program, which offers immediate access to a Counsellor or Caseworker every Thursday from 9 AM to 4:30 PM. Call ahead to book an appointment, or Walk-in.
Referrals are not required and this program is open to the public. This program is free of charge; funded by the SE LHIN, United Way of Leeds and Grenville, and the Ministry of Health and Long Term Care.
